JR. XTREME HOCKEY SCHOOL

Refund Eligibility: Refunds may be requested up to two (2) weeks prior to the start of the camp. No refunds will be issued after this deadline.

Refund Process: To request a refund, participants must contact us at mdewar@naxhockey.com. Please include your full name, registration details, and reason for cancellation in your request. Refunds will be processed via e-transfer within [number of days/weeks, e.g., 7-10 business days] from the date of cancellation approval.

Refund Amount: If eligible, the refund amount will be the full amount of the registration fee paid, minus a $40.00 administration fee

Non-Refundable Circumstances: Registrations cancelled after the deadline specified above will not be eligible for a refund, except in cases of documented medical emergencies or other exceptional circumstances as determined by the tournament organizers.

Event Cancellation: If the camp is cancelled due to reasons beyond our control (e.g., inclement weather, safety concerns), participants will be notified promptly, and a full refund will be provided.

Participants acknowledge and agree to abide by this refund policy by registering for this Devon Jr. Xtreme Camp.

I, acknowledge that they will participate in the Devon Jr. Xtreme Camp. I recognize that there are risks of injury while playing hockey.

I, for myself, my heirs, executors, personal representatives, administrators and assigns, hereby release Devon Xtreme, its board of directors, officers, employees, servants, agents, representatives and volunteers (NAX Releases) from all claims, demands, damages, actions or causes of actions arising out of or in consequences of any loss, injury or damage to person or property incurred while attending and participating in the Devon Jr. Xtreme Camp.

I understand that it is my responsibility to keep the team medical trainer advised of any changes to my medical information throughout the camp. In the event of a medical emergency and that my emergency contact cannot be contacted, the team medical trainer will arrange to take my child to the hospital or a physician if deemed necessary. I hereby authorize the physician and nursing staff to undertake examination, investigation and necessary treatment of my child. I also authorize the release of information to the appropriate coach and/or physician as deemed necessary.

By signing this form you are granting the therapists and health professionals at Northern Alberta Xtreme to share information that may arise during the evaluation process. These may include personal physicians, sports medicine physicians, athletic therapists, physiotherapists and chiropractors.

By signing this form, you are also granting consent for the medical staff at Devon Xtreme to give their professional advice and care for the athletes.
